JUSTIFICATION: Une hémolyse provoquée par de fortes doses d'acide ascorbique (AA) chez des patients présentant un déficit en G6PD a été rapportée, mais elle est rare. À notre connaissance, il s'agit du premier cas signalé chez un homme présentant un déficit en G6PD, coexprimant avec une cholécystolithiose et une cholécystite, ayant développé une hémolyse extrême et une hyperbilirubinémie après avoir reçu une perfusion de doses d'acide pharmacologique d'acide ascorbique. PRÉOCCUPATIONS DES PATIENTS: Un homme de 27 ans présentant un déficit en glucose-6-phosphate déshydrogénase a été admis à notre hôpital en raison d'une cholécystolithiose et d'une cholécystite. Il est apparu avec une jaunisse sclérale et une urine très colorée après avoir reçu une perfusion de doses pharmacologiques d’acide ascorbique. DIAGNOSTIC: Les résultats cliniques, combinés à ses antécédents médicaux et à divers résultats de laboratoire, ont confirmé le diagnostic d'hémolyse et d'hyperbilirubinémie induite par l'acide ascorbique. INTERVENTIONS: Le patient a été traité avec des stéroïdes, des médicaments hépatoprotecteurs et de l'acide folique, en plus d'éviter les agents présentant un risque connu d'hémolyse (comme la vitamine C). RÉSULTATS: En conséquence, les symptômes de la jaunisse hémolytique se sont améliorés, l'hémoglobine est restée stable et la patiente a obtenu son congé 11 jours plus tard. ENSEIGNEMENTS: Les cliniciens doivent garder à l'esprit la possibilité qu'une exposition à la vitamine C puisse entraîner une hémolyse chez les patients présentant un déficit en G6PD, en particulier chez les patients atteints d'une maladie grave connue.
