L'acide ascorbique à haute dose (HDAA) a été présenté pour améliorer l'inflammation et réduire les besoins hydriques au cours de la réanimation après choc de brûlure (BSR). On ignore si cela mènera à de meilleurs résultats. L'objectif des auteurs pour cette étude était de comparer les jours de ventilation, la pneumonie associée à la ventilation et la mortalité entre les patients qui ont reçu ou non de la HDAA pendant la BSR. Les auteurs ont mené une étude cas-contrôle rétrospective de 2012 à 2015. Ils ont identifié 38 patients ( HDAA) qui ont reçu HDAA pendant la BSR. En utilisant l’âge et le pourcentage de TBSA, les auteurs ont identifié et comparé 42 patients témoins (CTL) qui n’avaient pas reçu de HDAA pour BSR au cours de la même période. Les auteurs ont recueilli des données sur l’âge, le pourcentage de TBSA, les jours d’hospitalisation, les jours de ventilation (VENT), les lésions par inhalation (INH), la pneumonie associée à un ventilateur (PAV) et la mortalité (MORT). Il n’existait aucune différence d’âge et de% TBSA ou% TBSA de brûlures au troisième degré entre les groupes. Il n'y avait pas de différence significative dans l'incidence de l'INH (HDAA-52% par rapport aux CTL-36%, p = 0,17) et les groupes présentaient une DS et un VENT similaires. En outre, il n'y avait pas de différence significative dans l'incidence de la PAV (HDAA-29% par rapport aux CTL-14%, p = 0,13) ou la mortalité (HDAA-26% par rapport aux CTL-23%, p = 0,8). Les patients atteints d'HDAA présentaient une incidence numériquement plus élevée d'insuffisance rénale aiguë nécessitant une dialyse (23 vs 7%, p = 0,06), ce qui a été confirmé par une analyse multivariée (odds ratio 5,4; intervalle de confiance à 95% 1,1-26). La HDAA, bien qu'elle réduise potentiellement les besoins en inflammation et en liquides pendant le BSR, pourrait ne pas améliorer les résultats significatifs tels que les exigences en matière de ventilation, la pneumonie associée à la ventilation et la mortalité.
