Bien qu'il ait été démontré que les ccRCC avaient une méthylation aberrante de la cytosine et une perte d'hydroxyméthylation (5hmC), l'impact pronostique et le ciblage thérapeutique de cette aberrance épigénétique n'ont pas été complètement explorés. L'analyse de 576 échantillons primaires de ccRCC a montré que la perte de 5 hC était associée de manière significative aux caractéristiques clinicopathologiques agressives et constituait un facteur pronostique indésirable indépendant. La perte de 5hmC a également prédit une survie sans progression réduite après résection de la maladie non métastatique. La perte de 5hmC dans les ccRCC n'était pas due à l'inactivation des enzymes TET par mutation ou transcription, mais par leur inactivation fonctionnelle par le l-2-hydroxyglutarate (L2HG) surexprimée en raison de la suppression et de la sous-expression de la l-2-hydroxyglutarate déshydrogénase. (L2HGDH). L'acide ascorbique (AA) a réduit la méthylation et restauré les niveaux de 5hmC à l'échelle du génome via l'activation du TET. L'extinction de fluorescence de la protéine recombinante TET-2 n'était pas affectée par le L2HG en présence de AA. Le traitement pharmacologique par AA a entraîné une réduction de la croissance du ccRCC in vitro et une croissance réduite de la tumeur in vivo, avec une augmentation de 5hmC intratumorale. Ces données démontrent que la réduction de 5hmC est associée à une réduction de la survie dans les CCRCC et fournit une justification préclinique pour explorer le potentiel thérapeutique des AA à forte dose dans les CCRCC.
